Adding a Dependent

Complete an Affirmation for Eligible Dependent(s) form to add your dependent to the system (even if you are not adding them to any benefits coverage at this time). If you are adding them to benefits coverage, the Affirmation is part of the Benefit Change Form 857. Dependents can be added to coverage within 31 days of a qualifying event, such as birth, adoption, or being enrolled as a full-time college student. Pay special attention to the eligibility requirements for children in the different benefits plans, as explained on the Affirmation form. Completed forms can be mailed to the Marathon Oil Benefits Department, Health & Welfare Benefits, 5555 San Felipe Street, Houston, TX 77056, faxed to 713-513-4495, or scanned and emailed to MROBenefitsHelp@MarathonOil.com.

If You Need to Change… Then You Need to…

Federal and/or state tax withholding

Go to HR Central, then select the action tab drop down under 'My Self' and choose 'Payroll and Time.'

Health, Dental and/or Vision coverage

  • If this is your first child, you need to compare the health and dental and vision plans offered by the Company to that of your spouse’s employer, to determine who should carry the dependent coverage.
  • Compare coverage, deductibles, coinsurance/co-payments, monthly contributions, additional charges/ penalties such as late cancellation charges, and determine how benefits are coordinated.
  • If you and your spouse are both employed by Marathon Oil, be aware that only one of you can insure eligible children, and that you cannot be a member and a dependent of the same plan.
  • To make changes to your Health, Dental or Vision Plan coverage levels, complete the Benefit Change Form within 31 days of the qualifying event. Fax it to 713-513-4495, mail it to the Marathon Oil Benefits Department at Health & Welfare Benefits, 5555 San Felipe Street, Houston, TX 77056, or scan and email it to MROBenefitsHelp@MarathonOil.com. If you miss the 31-day deadline, you will need to wait until the company’s Benefits Open Enrollment period and elect coverage for the following year.

Flexible Spending Accounts - Health Care Spending Account (HCSA), Limited Health Care Spending Account (LHCSA) and/or Dependent Care Spending Account (DCSA)

  • Review the amount you are contributing to the flexible spending accounts.
  • If you want to make a change to your current year HCSA or LHCSA contributions, complete the Benefit Change Form within 31 days of the qualifying event. Fax it to 713-513-4495, mail it to the Marathon Oil Benefits Department at Health & Welfare Benefits, 5555 San Felipe Street, Houston, TX 77056, or scan and email it to MROBenefitsHelp@MarathonOil.com. If you miss the 31-day deadline, you will need to wait until the company’s Benefits Open Enrollment period and elect coverage for the following year.
  • If you want to make a change to your current year DCSA contributions, complete the Benefit Change Form within 31 days of the qualifying event. Fax it to 713-513-4495, mail it to the Marathon Oil Benefits Department at Health & Welfare Benefits, 5555 San Felipe Street, Houston, TX 77056, or scan and email it to MROBenefitsHelp@MarathonOil.com. This must be completed within 31 days of the date the child is placed in day care, and you (and your spouse, if married) have returned to work.

Health Savings Account (HSA)

  • You may change your contribution amount at anytime of the year. To change your HSA contributions, please send an e-mail to MROBenefitsHelp@MarathonOil.com.

Accidental Death & Dismemberment (AD&D) coverage

  • If you are currently enrolled for coverage, you can add your child to that plan within 31 days of the qualifying event, using the Benefit Change Form.

Dependent Life Insurance coverage

  • If you are currently enrolled in Contributory Life Insurance coverage for yourself, you can add coverage for your child within 31 days of the child becoming an eligible dependent.
  • If this is your first child, you may want to add Dependent Child Life Insurance coverage of $10,000, $20,000 or $30,000.
  • If you are currently enrolled in Contributory Life Insurance coverage for yourself, you can add coverage for your spouse within 31 days of the spouse becoming an eligible dependent.
  • You may purchase coverage for your spouse in $10,000 increments up to $50,000. During the annual Benefits Open Enrollment, you may increase your spouse life insurance by $10,000 each year until you reach the $100,000 maximum.

Long Term Disability Insurance (LTD)

  • If you are not enrolled in the LTD plan, you can complete an application for coverage within 31 days of the qualifying event. To request the application, click here. Matrix (our LTD vendor) determines eligibility for participation in the plan.

Occupational Accidental Death Benefit (OAD)
